Empowering Ability, Enhancing Life

Real-world outcomes, personalised supports, and evidence-based care designed for your goals.

At Perfect Motion Physiotherapy, Adelaide, we deliver tailored physiotherapy services for NDIS participants living with complex disabilities, those who are bedridden or severely limited in mobility due to injury, illness, or chronic conditions. Our aim is to improve function, reduce complications, and support meaningful outcomes in line with the NDIS Outcomes Framework .

Who We Support

We care for participants with:

  • Bedridden or severely limited mobility
  • Neurological conditions (e.g., stroke, MS, cerebral palsy, ABI)
  • Spinal cord injuries, amputations or major trauma
  • Progressive conditions (e.g., muscular dystrophy, MND)
  • Chronic pain and functional limitations
  • Ageing-related disability support needs

Our service is mobile available in homes, residential facilities, aged-care settings, and hospitals.

What the NDIS Requires?

As an NDIS registered allied health provider, our reports must:

Explain the therapy approach, and provide evidence of outcomes and progress

Demonstrate progress towards participant goals, including barriers, adjustments, and evidence-based recommendations

Recommend reasonable and necessary supports aligned with the NDIS Principles: effective, beneficial, value for money, and independence-enhancing

NDIS

Our Therapy Approach

  • Initially: Comprehensive assessment covering physical, neurological, respiratory, pain, and skin integrity factors
  • Collaboratively: Establish NDIS-aligned goals using SMART principles and tools like Goal Attainment Scaling (GAS)
  • Evidence-based: Deliver interventions with proven effectiveness in populations with complex disability—always linking back to participant goals

Ongoing monitoring includes:

Functional outcome measures (e.g., FIM, manual muscle testing, ROM, CPAx)

Safety-related metrics (e.g., Braden scale for pressure risk)

Individualised goal tracking via GAS or Goal Attainment Scaling Light

Progress reports aligned with NDIS progress reporting requirements

Typical Interventions

Passive and active-assisted movement for ROM & contracture prevention

Therapeutic exercises to improve strength and mobility

Chest physiotherapy, breathing exercises, and positioning to prevent complications

Pressure-relief strategies and skin monitoring

Bed-mobility and transfer training using hoists/frame techniques

Pain management and comfort strategies

Assistive technology recommendations (e.g., pressure cushions, commode chairs)

Referral links to allied supports: OTs, dieticians, nurses, carers, GPs, and case managers
